GROUP ELIGIBILITY

- Minimum of 50 persons
- Only one (1) option per group
- Contestability period for pre-existing illness is one (1) year.

INDIVIDUAL ELIGIBILITY

- Must be associate member or dependents of the cooperative members
- 4–17 years old
- Exit age is on the 18th birthday
- Single policy per member per year per coop
- In good health, mentally competent and physically able

BASIC CLAIMS REQUIREMENTS
- Endorsement Letter from the cooperative
- Death Certificate (Original or Certified true copy from the original duly signed by the Local Civil Registrar)
- Proof of Beneficiary (Birth Certificate of child)
- Police Report or Affidavit of Eye Witness original copy (for accident claim)
- Claimant Statement
- Identification
- Attending Physician Statement (if died in the hospital) or Barangay Certificate or Investigative Report stating the place and cause of death (if died at home).
- Proof of Remittance & Payment from the cooperative.
