- Original copy of Barangay certification.
- Clear pictures.
- Original copy of certification from the Bureau of Fire Marshall.
- Duly notarized affidavit from the fire victim/owner or renter.
- List of damage.
- Estimated cost of damage.
- CLIMBS Nonlife Policy Number.
- Proof of payment.
- Cooperative application form or loan application.
